Ambulance Service and NHS Trust Admit Failures in Care Leading to Death of Ebow Graham, Foreign Beggars Founder
Ebow Enyan Graham, also known as Metropolis, a founding member of the hip-hop group Foreign Beggars, died in April 2020 after falling from a third-storey window in Clapton, East London, following a psychotic episode. Both the London Ambulance Service and East London NHS Foundation Trust have admitted breaching their duty of care in the events leading to his death and have reached an undisclosed settlement with Graham’s former partner, Francesca Macri.
On 17 April 2020, Graham’s friends called 999 at about 4:20 pm, concerned about his erratic behavior during a psychotic episode. When paramedics arrived, they dismissed the friends’ concerns and judged that Graham had capacity over his care without contacting mental health specialists, which was against protocol.
At 6:30 pm, Graham’s flatmate, a GP, alerted the mental health crisis team about ongoing serious concerns. However, the crisis team nurse failed to arrange an urgent mental health assessment within the recommended four-hour window, offering an appointment only the following morning.
Later, Graham’s friends took him to a hospital but left due to his agitation and restlessness. They made two further emergency calls expressing grave concerns. At 12:25 am on 18 April, a third emergency call reported Graham had fallen from a window. He suffered injuries and cardiac arrest en route to hospital and died shortly after arrival.
The London Ambulance Service’s investigation acknowledged several missed opportunities to obtain expert mental health advice and admitted failing to follow appropriate protocols.
East London NHS Foundation Trust admitted shortcomings in handling the crisis call and risk assessment.
Francesca Macri criticized the ambulance team’s dismissive attitude and questioned whether racial profiling influenced their response, as paramedics asked if Graham had smoked cannabis, seemingly focusing on the cause rather than his urgent need for help. She emphasized the need for swift and appropriate mental health treatment to prevent such tragedies in the future.
Macri’s lawyer highlighted the importance of health professionals listening carefully to loved ones in distress to ensure urgent assessments and appropriate care.
Daniel Elkeles, Chief Executive of the London Ambulance Service, expressed condolences and acknowledged shortcomings in care but strongly refuted claims of racial profiling. He stated that the trust has learned lessons and taken action since Graham’s death.
Ebow Graham was widely respected as a gifted MC and founding member of Foreign Beggars, whose unique blend of hip-hop, grime, and dubstep influenced the UK music scene. He was also a devoted father to his 10-year-old son, Cassius. Tributes from fellow artists and fans highlighted his warmth, talent, and generosity.
This case underscores critical gaps in emergency mental health care and the importance of proper risk assessment and specialist involvement in crisis situations, especially when patients exhibit signs of severe mental distress. The settlement and admissions by the ambulance service and NHS trust reflect acknowledgment of these failures and the urgent need for systemic improvements.
